The Case Manager (CM), reporting to the Senior Director Transitions and Case Management will work directly with clients and their family/support system of various diverse socio-economic backgrounds to establish rapport while facilitating wellness around complex physical and/or mental health conditions to improve quality of life. The CM works as a member of the CiA team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity and community.
Essential Duties:

1. Work with older adults in Contra Costa County to help improve their quality of life by developing care plans, facilitating access to resources, advocating for client rights, and promoting self-sufficiency and empowerment;
2. Build rapport with clients and families with diverse socio-economic backgrounds;
3. Conduct prescreening, home visits, assessments, phone calls and outreach linkages for clients; meeting program guidelines;
4. Build relationships with possible linkage/referral sources around healthcare, social services, mental health, housing, employment, and education;
5. Coordinate all necessary referrals/linkages on behalf of client needs;
6. Comply with all paperwork requirements, including maintaining accurate, current and concise records, such as - referral/intake screens, logs, progress notes, follow-up monitoring, demographic and other statistics, and meeting all program deadlines;
7. Submit required reports to Director in a timely manner, meeting all program deadlines;
8. Any other duties required by the Director.
Minimum Qualifications
1. High school diploma or equivalent, or associates degree in related field such as social work or psychology.
2. Bachelor’s level degree preferred, or equivalent experience in case management with older and/or disabled adults; nonprofit experience preferred.
3. Working knowledge of MS office, Google suite, and basic internet usage.
4. Experience working with the Older Adult population preferred.
5. Bi-lingual English/Spanish or English/Tagalog desired.
